Form Building

Within the setup of the blocks there is a new area in which you can attach a customized form. That form will be added to the tee time registration process. The forms are created directly in the Tee Time System.
  
After logging into the admin site of tee times, go to Tools > Registration Forms.
Select New Form button. If you already created some forms, then it will be listed last in the list.

You will see three tabs across the top:

  • Questions – This is where your questions will be listed and created.
  • Details – Go here to name your form, setup the date in which all answers should be responded by, attach the event reminder communication, and flag how many hours prior to the due date the reminder should go out.
  • Export Submission – This will give you a CSV export of the answers to the questions.

Along the left are the different types of questions you can create:

  • Text Field – are short one word answers
  • Text Area – you can write a paragraph
  • Check Box Field– check from a list
  • Radio Button Field– fill in the circle
  • Drop Down Field– Select one from the list
  • Html Area – Free form, use for lines, pictures, or headers

Left-click & hold on the type of question you would like to use, and drag to the Questions Section.

Click on the “e” to format your question.Click the "x" to delete the question.

Format your question:

  • Name - This area will list all your previously asked questions. If you select a previously asked question, the system will remember, who the question is for, if it’s required and give the potential answers. Furthermore, the system will remember how your member’s and guest’s answered them the last time and auto fill the answers when the member is prompted to answer, making it easier for them to fill out.
  • New Question Name – This will make and entry into the Name drop down for you and will allow you to format a new question.
  • Prompt - this is what the member/guest will see on the screen.
  • Question Is For - who should see the question
  • Is Required - This will inform the member when the answers need to be given. Immediately – at the time of signup; At a later date will prompt an admin to select when the answers are needed by, if overriding the “Details” section; Use Form Setting will use what was outlined in Details (above); Not Required – answer is not needed.

Hit Save button, then continue to add the rest of your question types.

Once you have questions, you may now stack your questions. This means you can make questions dependent upon answers from another question. For instance, you may ask the member if they would like a boxed lunch, if they answer yes, you can ask what type, if they answer no, you don’t ask the question. To do this:
In the setup of the question, go to Display Settings.
Check off When the user answers another question.
Section the question that needs to be answered first, then mark the answer that should prompt this question.
Hit Save button.
Once the event is opened, click on Advanced tab.
Now go to the block in which you would like to use registration for and click on Edit Block button.
Click on the Event/Lottery tab along the top of the Block detail page.
In the block go to the This Event requires players to fill out a registration form, select the appropriate form, then click on SUBMIT button in lower left corner of page. You have now attached a form to a block.

Important to remember! It might be good practice to use more forms instead of using the same one over and over, as the answers from the previous event will be listed on the export.
